Open JMeter's Plugins Manager (Options > Plugins Manager), find "Variables from CSV File" on the Available Plugins tab and click Apply Changes and Restart JMeter. From the command line, run "./PluginsManagerCMD.sh install jpgc-csvars" (or "PluginsManagerCMD.bat install jpgc-csvars" on Windows) in your JMeter bin directory. The plugin ID is jpgc-csvars.
